Patient's Query

Hello doctor,

I am a 34-year-old male and weigh 280 pounds. I have tried everything I could come across or thought would help to lose weight. However, nothing worked. I read of a BF1 genetic mutation that can result in persistent obesity. Is this true? How can I know if this is the reason behind my weight? Please help.

Thank you.

Hello,

Welcome to icliniq.com.

Your first query consult is on us!

Your concern is valid. Obesity occurs due to high carbohydrate intake with less physical activity. It can be due to drastic hormonal changes as well, which are related to your current age, as you are 34 years old. We need to find out the cause of obesity first, its effect where the body hits metabolic lock due to a lot of trial and error methods. It can also be genetically also but that can be only 15 to 20 percent.

I suggest not making any drastic life changes, or restricting diet in a way you can not sustain. If you are trying to achieve a sustainable healthy living, then follow this. A sudden exercise routine will not help. I recommend starting with small goals like 3000 steps a day or having four glasses of water. It can be customized as per your lifestyle and needs.

Check for your blood reports like hemoglobin levels, which can lead to low energy levels and low metabolism, leading to weight gain. I suggest checking for the obesity stage by calculating your BMI (body mass index). Identify lifestyle changes or stress factors. I require more details to assist you with weight loss. I suggest the following:

1. Balanced carbohydrate intake throughout the day.

2. Do not skip meals. Try not to let more than four to five hours go between meals or snacks, to maintain stable blood sugars and diminish extreme hunger.

3. Gradually increase intake of high-fiber food.

4. Physical activity is very important. Go for at least 20 minutes of walking daily.

5. Include high-fiber carbohydrate food like whole grain bread; whole wheat pasta, brown rice, high fiber cereals, oats, barley, legumes (peas, beans, and lentils), corn, bran, seeds, and nuts; and fresh vegetables like broccoli, cauliflower, carrots, apple, and pear.

6. Emphasize lean protein foods at 15 to 20 percent of total calories. Try to include protein in most meals and snacks by including dal, legumes, chicken, fish, and low-fat dairy products.

7. Consume about 25 to 30 percent of calories as fat, emphasizing low saturated fat foods and increased monounsaturated and omega-3 fatty acid food choices. Mono unsaturated fat-containing food choices include avocado, olive oil; olives, and nuts such as almonds.

8. Omega-3 fatty acid (specifically EPA, which is Eicosapentaenoic acid, and DHA, which is Docosahexaenoic acid) containing food choices include cold-water fish such as salmon, mackerel, halibut, tuna, sardines, and herring. Include seeds like flax seeds, chia seeds, and walnuts.

Foods to avoid include:

1. Refined carbohydrates cause inflammation, exacerbate insulin resistance, and should be avoided. These include highly processed foods such as white bread, muffins, semolina, and pasta.

2. Limit food containing trans fatty acids, especially deep-fried foods like chips or canned products.

3. Avoid red meats as they are high in saturated fats.

I hope this information will help you.
