Patient's Query

Hi doctor,

I have severe mood swings. I am also depressed. I prefer to have a monk-like life. I want to travel around the world. I cannot bear the pain of animals on the streets. I was diagnosed with chronic kidney disease six months back. But I have had mood swings for the last six years after the death of my father. I have no interest in money. I took Escitalopram for almost five years along with Propranolol. I find it unable to adjust with my mother. Currently, I take Prazosin 5 mg BD(twice daily), Telmisartan 80 mg OD(once daily), Dytor 10 mg OD(once daily), Amlodipine 10 mg OD(once daily), Febuxostat 40 mg OD (once daily), Iron supplement, antacid Sodium bicarbonate. I had taken Nexito 5 mg and Propranolol 10 mg previously for the same complaint. Please help.

I understand your concern. In my opinion, a change in medication along with non-pharmacological methods is required. As far as medicines are concerned, I suggest increasing the dose of the tablet Nexito (SSRI) as the dose you take is insufficient. You can also discuss adding a mood stabilizer. I suggest the following non-pharmacological methods;

1. Practice relaxation techniques (like Breath relaxation exercises) daily.

2. Spend some me-time (i.e., give at least one hour daily for your hobbies).

3. Share your feelings with family members or close friends. It relieves your burden.
