Why is there difficulty in losing weight in PCOS patients?

Hello doctor,

I am concerned about a possible hormonal, adrenal, or thyroid issue. My gynecologist and PCP dismissed my symptoms last year, and I cannot see an endocrinologist until two months, but I would like to get healthier sooner. I included my current symptoms, medications, and last week's blood work reports. I am a 30-year-old female of height 5.4 feet and weight 230 lbs. I am taking Spironolactone 100 mg per day (50 mg for three years, then raised to 100 mg three months ago). In transvaginal pelvic ultrasound taken last year, there were no abnormalities reported.

My symptoms are difficulty in losing weight when eating 1200 calories a day (weighing portions), whole-body inflammation with joint pain that worsens when eating carbohydrates, edema in the lower legs and feet, hormonal acne, cold hands and feet, anxiety and irritability, an adrenaline rush or fight or flight panic every afternoon like clockwork between 3:00 to 7:00 PM that comes from my lower chest with no triggers, rash or hives on arms and chest, long light periods for two weeks on and two weeks off, and lower left pelvic or ovary pain during ovulation.

What causes fluctuations in the blood sugar and cholesterol levels in a diabetic patient?

Hello doctor,

I have had diabetes for the last 15 years and taking regular medicines. For the last two years, I have been taking the following medicines, Reclimet-XR 60/500 mg twice daily, Deponorm 5/500 mg twice daily, and Ecosprin AV 75/20 mg once, but around 20 days back, my sugar was going down to almost 30 to 40 mmHg. So, I consulted another doctor around 15 days back who gave the following medicines, Vildagliptin-M 50/500 mg twice daily and Ecosprin AV 75/20 mg once, but my fasting sugar is ranging around 125 mmHg and PP around 145 to 150 mmHg which is worrying. Can you please advise me on the medicines which are best suited for my condition and my age, that is, 70 years? I am attaching herewith my latest blood test report, which is indicative that my good cholesterol is 33 mg/dL and platelets 105, which I feel are low.

Which medicine should be taken to treat high levels of homocysteine?

Hello doctor,

This inquiry is for my sister-in-law. Three four years back, she had a problem with her eye. On consulting, they told due to some type of stroke, her vision has been affected. They started giving him Ecosprin, and a special injection to protect her eye vision. During this period her retina specialist suggested she have homocysteine reading at regular intervals. She had the test and her reading was found to be 19.7 H (range is 0.0 to 15.00). She is non-diabetic, has no hypothyroidism, and has no hypertension. At present she is taking the following medicines, Ecosprin, Shelcal, and Neurobion, Her gynecologist advised her Primosa and Evion capsules, as they are seeing some fibroids in a mammogram. My query is:

1. Which medicine she has to take for high homocysteine and for how many days and when it has to be repeated?

2. How this hormone is related to eye problems, as it was advised by a retina specialist? Please guide.

Is it possible to have hyperthyroidism and increased radioiodine uptake without any symptoms?

Hi doctor,

I had body aches, tiredness, and weakness for several months. Two months ago, I went for a blood investigation to rule out the cause. I was diagnosed to have hyperthyroidism from my blood reports. But Graves antibodies came negative. They advised me to do a radioactive Iodine uptake thyroid scan after one month, and it showed increased uptake. But I did not have any symptoms and was perfectly fine. I was asked to repeat my blood test, and this time my thyroid levels were within the range and without any medication. My doctor suspects this to be postpartum thyroiditis.

I want to reduce my weight as I am in prediabetes stage. Please help.

Hello doctor,

I was told I was prediabetic about a year ago during a hospital visit. My HbA1c level was 5.3 %. I have tried to lose weight on my own by changing my diet, and at first, I was doing well with reducing calories and walking. Then at around the three-week mark, I fell off the wagon when I only saw one to two pound weight loss. I think the inconsistency is doing more harm to my body than anything. Is it a good idea to see a specialist to test and confirm for diabetes and ask about medications or options to help me with hunger and cravings?
