Hello doctor,

I have had diabetes for the last 15 years and taking regular medicines. For the last two years, I have been taking the following medicines, Reclimet-XR 60/500 mg twice daily, Deponorm 5/500 mg twice daily, and Ecosprin AV 75/20 mg once, but around 20 days back, my sugar was going down to almost 30 to 40 mmHg. So, I consulted another doctor around 15 days back who gave the following medicines, Vildagliptin-M 50/500 mg twice daily and Ecosprin AV 75/20 mg once, but my fasting sugar is ranging around 125 mmHg and PP around 145 to 150 mmHg which is worrying. Can you please advise me on the medicines which are best suited for my condition and my age, that is, 70 years? I am attaching herewith my latest blood test report, which is indicative that my good cholesterol is 33 mg/dL and platelets 105, which I feel are low.

I suggest the following:

1. Continue Vildagliptin-M (Vildagliptin and Metformin) 50/500 mg twice daily.

2. Take Ecosprin-AV (Atorvastatin and Aspirin) 75/10 mg once daily.

3. If you do not have any problems with urinary infection, then take Dapanorm (Dapagliflozin) 10 mg daily.

4. Repeat the platelet count again. If low, meet with a hematologist.

5. Repeat HbA1c (glycated hemoglobin), FBS (fasting blood sugar), and PPBS (post-prandial blood sugar) after three months.

6. The cholesterol levels are fine, except for low HDL (high-density lipoprotein). Increase your exercise time and have more vegetables in your diet.

Thanks for the reply.

Please confirm the time of taking Vildagliptin and Ecosprin, that is, before or after meals, and Dapanorm once or twice daily and before or after meals.

Thanks again.

As advised, I am taking medicines now, but yesterday I got the three tests done, and the results are as follows, vitamin D 17 nmol/L, HDL 32.40 mg/dL, and platelets 100. I am getting a little worried about the very low Vitamin D. Please advise any medicine that can raise my vitamin D level.

I recommend you take syrup Arachitol Nano (Cholecalciferol) 60000 units weekly once for eight weeks and then monthly once for four months for vitamin D deficiency. For the low platelets, please visit a hematologist.
