Hello doctor,

I am 45 and weigh 298 pounds with a BMI of 44.2. I am desperate for help with severe obesity that is destroying my health. My recent labs show A1C at 9.4 percent, indicating poorly controlled diabetes, triglycerides at 420 mg/dL, and HDL cholesterol only 26 mg/dL.

My blood pressure is dangerously high at 168/102 despite being on three medications, such as Lisinopril 40mg, Amlodipine 10mg, and Metoprolol 100mg twice daily. I have tried every diet imaginable over 20 years, but always regain the weight plus more.

My knees are bone-on-bone arthritic, and I can barely walk up one flight of stairs without severe pain and shortness of breath. My sleep study showed severe obstructive sleep apnea with an AHI of 65 events per hour and oxygen desaturation to 76 percent.

I am using CPAP, but still feel exhausted. I have developed non-alcoholic fatty liver disease with ALT at 89 U/L, and my cardiologist says I am at an extremely high risk for a heart attack.

My endocrinologist mentioned bariatric surgery, but I am terrified of complications and the permanent changes. Are there new weight loss medications like Wegovy or Zepbound that might help? My insurance requires six months of supervised weight loss before approving surgery.
