Diagnosis and treatment of mental illnesses. Could you please answer a few questions?

I am doing research regarding diagnosis and treatment of mental illnesses. Please answer a few questions of mine. What routine medical examinations apart from MMSE and history would you conduct? By this I mean standard routine blood investigations plus other investigations. What clinical signs would suggest performing MRI scan and would you order it with every patient? What clinical signs would suggest performing thyroid hormone tests? Would you do the thyroid hormone test with every patient admitted to the ward? Would it be a standard psychiatric procedure? What neurological and physical examinations would you conduct to rule out any other medical or neurological disorders? Schizophrenia is a dysfunction of brain’s physiology; the functioning through neurotransmitters and through hormones but actually genetic problem is always there; means there is a dysfunction at cellular level. Which neurotransmitters and hormones would be responsible for this dysfunction? What kind of dysfunction of cellular level is that? What does an elevated level of homocysteine found in schizophrenic patients tell the psychiatrist?

My mother is restless, and has hallucinations and a small bump on her head. What to do?

Hello doctor,

This is on behalf of my mother, aged 82. She does not have sugar, blood pressure, or cholesterol and takes food normally. Four years ago, she underwent a major surgery sleeve resection for gist in the stomach. The operation was a success, and the tumor was removed. The postoperative pathology report showed some traces of not-so-malignant carcinoma. The surgeon sent her to a regional cancer center for review. The RCC advised her to take the tablet Imitab for one and a half years. After that, she has been visiting the RCC for a periodic check once in three months. Nothing unusual has been found so far.

Now my mother is experiencing certain other problems. She is always restless. She never lies down or sleeps in the daytime. During the night, she is very reluctant to lie down or sleep. She walks all night and talks to persons who are not present as if in a hallucination. The doctor said it can be early dementia and has advised an evaluation of intracranial pathology for generalized hyperpigmentation.

For the past couple of days, her restlessness and hallucinative visions, and talk increased alarmingly. She even started climbing the windows at night. We shall be much obliged to you if you, please recommend some medication to control this behavior temporarily till the test is taken and regular treatment is started.

I have low sexual drive with occasional morning erection. Please help.

Hello doctor,

I am a 30-year-old male. I have been married for one year. My problem is low sexual drive. I do have morning erections but not regularly. When I have sex one to two times a week, I do not masturbate. Otherwise, I masturbate two to three times a week. Unlike having sex, I get enormous pleasure when masturbating. When masturbating, I withdraw for a few seconds when I am about to ejaculate so that I get a long session of masturbation and also a good climax. When having sex I do not get the same pleasure when sliding my penis inside her.

I guess I have a low sexual drive from the past one and a half years. I do not feel any arousal when I see her naked. But when I watch a porn movie, I get aroused sooner. I consume Himalaya Gokshura capsules for vitality for the past one month. Apart from that no medications.

I am a strict non-smoker and non-alcoholic. I do have some stress out of workplace and family issues. I do not get enough sleep. I did my circumcision when I was 7 years old. My penis is dry and I feel less sensitivity. When having sex, I normally ejaculate within five to ten minutes after insertion. No medical conditions like seizure of fits. But I feel tired at times.

I had been watching porn for the past several years. Could that be a reason for my low sexual desire? Or is it masturbation? I had consulted a doctor in the icliniq. She prescribed me Bupron XL 150 mg for 15 days. I understand that it is an antidepressant. Will it help to increase my libido? Are there any short term or long term side effects? Can I use this pill on a regular basis? Please help me to overcome this situation.

I lost my energy and power due to bad masturbation. How to regain it?

Hello doctor,

I am 29 years old. I masturbate badly (without lubricants) for over 13 years. Because of this, my artery becomes loose and both of my scrota are becoming small. When I work I get muscle pain, back, and leg pain. I lost my energy and power. I want to recover my energy as soon as possible because I am going to get married within three to four months. Please help. I have not taken any medication yet.

Does CBT help in relieving stress?

Hi doctor,

I wanted to know about CBT in a brief. How effective is it? If possible please provide me links to some books for the same. Do I need a psychiatrist to help or can I practice on my own with such self-help books? I have read that almost all patients reduce their symptoms drastically when those techniques are practiced sincerely, and program their brain to be calm. I am not a pop-a-pill type of person, I always look for some natural methods before I start any medication. I do not have any serious issues. I just wanted to try it out for stress instead of SSRIs. I also try dark chocolate, tea, walking, Vitamin B, etc. and it is helping. Please advise.
