What could cause iron deficiency or iron over-absorption?

Hi doctor,

My TIBC 508, transferrin 363, transferrin saturation 17%, iron 85, and total LDH 109. Do these values indicate iron deficiency or over-absorption? I am on regular Synthyroid, Singulair, Prilosec, vitamin C, and oral contraceptives.

High ferritin in thalassemia major: Management?

Hello doctor,

I am a thalassemia major individual with factor XIII deficiency, HCV+. I transfuse platelet or FFP daily and transfuse RCC weekly. All these things are reacting in my body. My ferritin level is 15000 after chelation therapy. This also has a bad effect every time. I am 28 years old. How long can I expect to live? Kindly help.

What is the treatment for microcytosis with low reticulocyte and hemoglobin?

My report says reticulocyte count is 0.2 %, hemoglobin is 4.2 g/dl, and RBC shows cytopenia with a dimorphic picture showing moderate microcytic hypochromic with mild microcytosis. What should be the line of treatment? Please suggest.

Is Xarelto safe for DVT in heterozygous factor V Leiden?

Hello doctor,

My girlfriend is 26 years old and was diagnosed with heterozygous factor V Leiden and heterozygous prothrombin 20210. She is very active, runs half marathons, and works out three to five times a week. She had broken her ankle, and we watched TV for two weeks and noticed some swelling. We took her to the doctor, and she had a DVT. She was also on birth control at the time. We have stopped birth control, and she did a regime of Xarelto.

Now since the doctors have found out, she has these two mutations, they want to put her back on Xarelto for life. Is this a viable option? Can she be safe without the blood thinners? We believe the clot was due to the increase factors (birth control, break, and sedentary life). Do you think she needs blood thinners for life or she can take then at times of increased risk breaks, surgery, pregnancy, etc.?
