Hello doctor,

I have chronic pain at the semimembranosus insertion points in both legs. I have had this for nearly 18 months. I am a runner who over-pronates. More recently I have had tightness and hyper irritation in my lower legs - shins, and calves. I have had a lumbar MRI for a bulging disc which showed a very slight disc bulge at L4-L5 on the left side but no impingement on the nerve. So, considered negative for pain in my legs. My question is, could the semimembranosus insertion point be affecting the tibial nerve and causing the tightness and irritation in my lower legs? This does not respond to stretching which often makes it worse.

I have understood your concern about the pain in the lower leg, semimembranosus.

See, semimembranosus is the muscle of lateral hamstring. If you feel pain exactly at the tendon of it, our treatment will be different, like ultrasound therapy. As you mentioned you are a runner, according to me, your hamstring and calf may be shortened or tight.

So, I suggest you go for physiotherapy treatment where you can be treated with ultrasound therapy of semimembranosus muscle tendon and simultaneously mild stretching of lower leg muscles, that is, calf muscle and all the hamstrings which would prevent recurrent pain.
