Why do I have a mild burning sensation in my penis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I sometimes feel a mild burning sensation in my penis, about two inches from the tip. I have noticed that if I have sex daily and ejaculate, I do not get this burning sensation.

However, if I do not have sex for three to four days, the burning returns. When sex is not possible at least once every two days, I masturbate to ejaculate because that reduces the burning. I want to understand the reason behind this and whether it indicates any problem.

I feel that when semen remains inside for two to three days, it causes this burning sensation.

Otherwise, I have no other issues. I have good erections, good sexual interest, and as a couple we enjoy sex fully with multiple orgasms and ejaculations. The only problem is the penile burning.

Please help.

Hi,

Welcome to icliniq.com.

I understand your concern.

If there is no history of burning during urination, any abnormal or foul-smelling discharge, purulent discharge, or multiple sexual partners, then this explanation is likely.

A mild burning sensation along the penile shaft (about two inches from the tip, corresponding to the distal urethra) that appears after three to four days without ejaculation and resolves quickly after sexual activity or masturbation is typical of congestive prostatodynia or, more broadly, chronic non-bacterial prostatitis (also known as chronic pelvic pain syndrome). This is a common, non-infectious inflammatory condition affecting the prostate gland and surrounding pelvic floor structures.

The prostate produces a major portion of seminal fluid, which mixes with sperm during ejaculation. When ejaculation is infrequent, prostatic secretions can stagnate and become thicker or more concentrated. This buildup irritates the prostate and the adjacent urethra, causing the localized burning or discomfort you experience.

If this is troubling you, continuing to ejaculate every one to two days is a safe and effective strategy. Staying well hydrated (about two to three liters of water daily) helps keep the secretions less viscous. Avoid caffeine, alcohol, and spicy foods if they seem to worsen the symptoms. Pelvic floor relaxation exercises and warm baths can help reduce congestion.

You should consult a urologist if symptoms worsen or if burning during urination develops. A per rectal examination and urine microscopy may be required for further evaluation.

I hope this has helped you.

Please feel free to reach out to me again for further queries.

Thank you.
