How to treat arthritis-associated neck and back pain?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I was recently diagnosed with arthritis in my C7 neck area. I experience constant pain in my neck, shoulder, and back, all on my right side. Physical therapy provides only temporary relief for a brief period. Each morning, I wake up stiff and sore, often accompanied by headaches, and lately, I have been having trouble sleeping. Additionally, I have been experiencing radiating pain down my right arm. The daily pain has become unbearable. While I try to limit my painkiller intake, they seem to have little effect, especially at night. I am currently taking the pill, antidepressants, and blood pressure medication. I am a 47-year-old female. I have previously undergone Cortisone treatment for shoulder bursitis. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

I suggest you undergo a neck MRI (magnetic resonance imaging), as your symptoms suggest potential neural involvement, possibly due to disc issues.

I suggest frequent application of ice to your neck and maintaining proper posture to avoid exacerbating your condition, such as prolonged neck bending. It is crucial to support your neck carefully while sleeping and to incorporate neck stretching exercises into your routine. Also, I suggest providing the MRI neck report to discuss further care and management options.

I hope this helps.

Please revert to assist you further.

Thank you.

The Probable causes

The probable cause is poor posture.

Investigations to be done

Investigations to be done are MRI (magnetic resonance imaging) neck.

Differential diagnosis

The differential diagnosis is

1. Facet joint arthropathy (spine joint degeneration causing pain and stiffness).

2. Radiculopathy (nerve root compression leading to pain, numbness, or weakness).

Probable diagnosis

The probable diagnosis is IVDP (intervertebral disc prolapse), where the soft inner part of a spinal disc pushes through a tear in the outer layer, often causing pain and nerve compression.

Treatment plan

For treatment, I suggest rest, ice, and disc reduction exercises.

Preventive measures

For prevention, it is necessary to maintain posture.

Regarding follow up

Follow-up will be done after an MRI (magnetic resonance imaging) neck with the report.
