Hello doctor,

I have some concerns about the test results. I assumed I had a urinary tract infection because of the pain in my urinary system. I visited the urgent care center, and the doctor did a dipstick test that revealed blood and protein. It was suggestive of a urinary tract infection, she noted. She submitted the urine sample for additional testing to identify the bacteria and prescribed Macrobid. The laboratory did not specify the antibiotic for her to give. Still, the data showed that they had found non-uropathogenic gram-positive organisms with more than 100,000 CFU. She advised me to get a swab test for vaginitis. Both tests were negative. Kindly help.

Regarding your results, the gram-positive non-uropathogenic organism is usually the bacteria that colonize the female urinary tract or vagina. For that, your doctor asked for a swab. You should continue the antibiotic for UTI for seven days, and it is good that other infections like vaginitis and STDs are harmful. Thank you for the reply.

I took the Macrobid, but it did not help. I am still experiencing symptoms and a lot of discomfort. So this is a UTI, and what antibiotic should I be on? Kindly help.

If Macrobid (Clarithromycin) did not work for you, then are two possible reasons:

1. Your UTI was not treated, and you need a different antibiotic, such as Ciprofloxacin.

2. Post-infection urethral syndrome after recently having a UTI. This is because the urethra can be very sensitive while recovering from infection, and it usually takes a few days to improve.

Suppose you are still experiencing the symptoms and have no improvement after seven days of the antibiotic course. In that case, I highly suggest you repeat the Urine analysis and cultures only with no need for an STD test or Vaginal swab again. Just to ensure no new organisms that might be resistant to Macrobid appear. And if a new organism appears, a list of susceptible antibiotics will appear to direct therapy.
