How to treat fatigue or loss of energy and feelings of worthlessness or guilt?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 12-year-old girl, and I am on a growth hormone injection and estrogen patch. I have the symptoms of fatigue or loss of energy and feelings of worthlessness or guilt every day. I have impaired concentration, indecisiveness, insomnia, or hypersomnia (excessive sleeping) almost daily. Markedly diminished interest or pleasure in almost all activities nearly every day (called anhedonia, this symptom can be indicated by reports from significant others). I also have restlessness, a feeling of slowed down, recurring thoughts of death or suicide, and considerable weight loss or gain (a change of more than 5 % of body weight in a month). I have attempted suicide two times already. Kindly help.

Hello,

Welcome to icliniq.com.

I am sorry to hear about your situation, and I understand it is difficult for you. What you are describing are symptoms of depression. First, you need to consult a local psychiatrist and start medication, as it cannot be prescribed online. Secondly, start counseling therapy like cognitive behavioral therapy and psychotherapy. I will help you cope with your negative thoughts and so you will be able to learn techniques to control your mind from going into the loop of negativity. Also, because you are already undergoing treatment for a long time, it can impact your physical and mental health. So, it would help you if you had a permanent therapist with whom you could talk every week or maybe once in 10 days, to help you keep track of your symptoms and to be able to take care of yourself. You can take therapy sessions from a local therapist, or you can opt for online sessions that are equally effective, convenient, and economical. I hope this helps.

Thank you.
