Hello doctor,

I got married five months ago. My husband and I have planned for a baby in our first month. But I could not conceive for the past months. Even though we have regular contact, I am not able to conceive. I had a preliminary checkup with a gynecologist and found my Hb (hemoglobin) low. Then I underwent treatment, and my Hb level is 12.6 g/dl now. The issue we face during contact is that the sperm fluid is not entering the tubes.

You have to be very patient to get pregnant. Five months for a young lady is too short for treatment. Hb (hemoglobin) is never a big cause of infertility. Hb (hemoglobin) up to 10 gm/dl is normal.

Do not worry about the semen not entering; the whole semen will not go in anyway. Sperms move up to the egg by chemical attraction, not physical. So if sperms reach the vagina, they will be attracted inwards. You can try having sex with putting your hip on a pillow. Then stay in the same position for half an hour with one or two pillows under. Turn to one side, left or right; this will not allow the semen to come out. Excess semen will fall out when you get up.

The quantity of semen is around 2.5 to 5 milliliters, but it contains only 0.5 milliliters of sperm. The rest of the semen is just a carrier. Semen gets the sperm to the cervix, which flows out once the sperm reaches the cervix. As soon as semen is ejaculated, it becomes thick and sticky so that it will stay on the cervix. After 15 to 30 minutes, the thick semen becomes liquid again, so after 30 minutes of sex, the sperm would have gone up, and the rest of the semen would flow out; this is the normal mechanism.

You seem very anxious to conceive. Here under is a checklist of investigations if not done can be done to find any problem. We usually give them after one year of marriage, but if you are anxious, you can try it.

  1. Your husband must go for two semen analyses done four weeks apart in an infertility center (an examination done by an andrologist) and blood sugars checkup.
  2. You go for a thyroid profile and blood sugar test.
  3. Confirm tubal patency. A fallopian tube connects the uterus and ovaries (the bridge where sperm meets the egg and forms a baby). This tube has the size of a hair follicle and cannot be seen on scans. So check with HSG (hysterosalpingogram), SSG (sonosalpingography), or laparoscopy.
  4. Nothing can stop a pregnancy if all this is done and found normal.
