Patient's Query
Hello doctor,
These migraines are ruining my career. The attacks start with a strange metallic taste in my mouth, then my vision narrows like I am looking through a tunnel, and finally, the pain hits—a vicious pounding on my right temple that nothing touches. Prescription triptans used to work but now give me chest tightness that terrifies me.
The worst part is the brain fog that lingers for days afterward—I have made embarrassing mistakes in presentations because I could not find simple words. My latest episode included numbness that spread from my fingers up my entire arm, and I panicked thinking it was a stroke. I have tried eliminating triggers, biofeedback, acupuncture, and even Botox, with minimal relief.
My neurologist mentioned a new CGRP (calcitonin gene-related peptide) inhibitor but warned it could cause depression.
Is this worth trying?
I am desperate for my old life back.
Please help.
Thank you.

Do not stop the medications abruptly without doctors' advice. Without knowing your complete medication history, it is not wise to advise any change or add any medication at this stage. But I would also request you to depend more on non-pharmacological methods.
Treatment plan:
Dietary supplements rich in magnesium, riboflavin (vitamin B2), and Coenzyme Q10.
