Hi doctor,

I got my period for the first time when I was 10. I started having serious issues with irregularity when I was about 14. I missed my period for months, so I went to the gynecologist, and he told me it was not a big deal, but if it bothered me, he could give me progesterone to induce my period and birth control pills to regulate it. That worked, but all the hormones made me feel horrendous. I stopped taking birth control pills, and within a year or so, my period regulated itself.

I am 18 now, and my period got irregular again, but I attributed that to the life change. I got it for two months on the same day, then fell ill with stomach problems that eventually went away and were attributed to IBS. I figured I would not get my period while having those issues because not only was I feeling bad, I was super stressed about school and my health. I got my period late a week after being sick with an odd illness (terrible nausea, weird chills and low-grade fevers) the doctor said was most likely a milder version of the stomach flu. I have not gotten my period since.

You are normal. Your period problem can be attributed to two different conditions at different times.

Normally a lady has her first period at 10 to 13 years, so you had it on time. But after that, it takes around 10 years for the hormonal cycle between the brain, pituitary, and ovary to mature. Till then, hormones will have ups and downs. So from 19 to 21 years, you can expect irregular cycles. As expected, you had regular cycles around that age, and everything went normal.

Now your periods are abnormal, not due to anything else but your weight.

From your history (attachments removed for patient identity protection), I presume you have PCOD (polycystic ovary disease), which is usually associated with obesity, irregular cycles, and no periods) and a few months later, heavy periods with clots and fleshy masses, spotting on and off, thyroid abnormality and prolactin problems indirectly.

You develop that extra pad of fat around the mid-segment of the body, especially the waist, thighs and breasts, with no fat beyond the elbow and knees. You have excessive body hair growth, hair fall, a dark shade over the lower half of the face, acne, oily face skin, and black skin over the back of the neck, inner thighs and under the breast surface. You may have a history of diabetes, especially your father or his family.

This will not allow ovulation to occur at a time, hence no periods. But this has a solution. Your weight has to be weight (in kilograms) = height (in centimeters) -100. Your expected weight is 52 to 56 kilograms. Once you reduce weight to normal range, you would not need any medicine to get periods.

To reduce weight -

  1. Do not do fasting.
  2. Do not eat junk food, fast foods, or red meat.
  3. Reduce snacks in between meals.
  4. Have low calories food.
  5. Eat food high in proteins.
  6. Do regular exercise, aerobics, yoga, and brisk walking.
  7. Try to reduce two to four kilograms per month. Do not hurry on weight reduction. You will see changes as soon as you reduce at least five kilograms.

    To have periods, take any combined oral contraceptive pills (packet of 21 pills) twice daily for 10 days. You will get periods 7 to 10 days after the last pill. This episode of no periods can also be due to some infection.
