Is HRCT of temporal bone sufficient for diagnosing lymph node enlargement?

Patient's Query

Hi doctor,

I just underwent an HRCT of the temporal bone, because I had a lump under my neck. I consulted an eye, ear, and throat (ENT) doctor, and he suggested to do HRCT of the temporal bone. No impressions were seen. I am worried about whether I need to do a separate scan or something, or does the HRCT of the temporal bone is fine. The symptoms include a lump of around 1 inch under my right ear, and another lump of 0.6 inch under my left ear. Both are hard. And those are pretty much the symptoms. I have never smoked. Doctor, I am extremely worried and have not slept well for a week. Please tell me what to do. I have attached the PDF.

Hi,

Glad to hear from you.

I am here to answer all your questions.

Thanks for providing the images and report (images are removed for patients identity). Reassuringly, the scan seems to be normal, with no ear disorders. The visualized parts of the brain and inner ear are normal, with no growth, tumors and no erosion of bones. However, this does not show the neck region to comment on the neck lymph nodes. Can you provide me with a detailed description of your symptoms and issues, and information on how you can find out the neck node? This would help in making a more accurate diagnosis and, if needed, an ultrasound scan of the neck to confirm the findings. The indication for the scan mentioned was tenderness of the mastoid tip, which is a bony structure and not a neck node. Could you kindly confirm your symptoms? Do you experience any difficulty in swallowing, changes in voice, or thyroid symptoms? Have you had any ear problems, discharge, or mastoid surgery in the past?

I am thankful that you have confidence in me to help you.

Please stay safe and healthy. Thank you.
