Hello doctor,
I am suffering from the following medical problems,
1) I have shortened plantar flexors.
2) Weakness of hip adductors or abductors of the hip in SLS, thus has to increase lateral translation.
3) Reduced balance while transferring body weight over a smaller BOS.
4) Weakened hip or trunk flexors or lower limb extensors.
5) Poor sensation and proprioception of the lower limb joints.
6) Shoulder pain.
7) Soft tissue adaptations due to immobilization.
Kindly suggest the tests required and the corresponding treatment for the same according to the functional problem.

I went through your query.
I have a few questions.
1. Is the patient independently mobile? If not, what aids are needed?
2. How much is spasticity a part of the problem?
3. Is the patient on night splints?
4. What therapy has the patient had, what has been the response, and any surgical interventions done or suggested?
I suggest the following,
1. Equinus is to be stretched with the maintenance of the night splints.
2. Hip strengthening and stretching of the iliopsoas.
3. Balance improvement with strengthening and hydrotherapy.
4. If sensory issues then protection of bone points and prevention of calluses are required.
5. Do an X-ray of the shoulder to rule out secondary arthritis or rotator cuff impingement.
6. Shoulder stretches and stability exercises to keep the head centered.
7. Do an MRI (magnetic resonance imaging) for a better assessment.
