Hi doctor,

I am a 24-year-old male. I have a hole in my tooth. After some time, part of the tooth got decayed and fell off, leaving the rest there. I have been having pain and taking Ibuprofen for the same.

I understand your concern.

I reviewed the pictures (attachments removed to protect the patient's identity). The destruction of the tooth is massive, having pain is very common in such cases because of the following:

1. If the tooth was not root canal treated before, the nerve of the tooth is still in it, and it is now near the oral environment (saliva, food, and drinks), which will cause pain.

2. Tooth margin after the breakage is not regular, and it becomes sharp enough to irritate the surrounding gums.

You have to visit the nearest dentist for clinical evaluation and have an X-ray to assess the remaining tooth structure, if it is enough to carry on treatment and preserve that tooth, or if extraction is the only way of treatment. Until visiting the dentist, Ibuprofen 600 mg every six hours or intramuscular injection every 12 hours if the pain is very severe. Avoid eating on that side, try not to have spicy food, and prefer to make your food and drinks at room temperature. Consult a specialist doctor and take medications with their consent.
