Hi doctor,

I am a 23-year-old female. I have a burning, urgency, and frequency feeling in my urethra all the time. Doctors tell me I have no UTI and that I need to see a urologist. I cannot get in to see one until three weeks. It started last week after having sex with my fiancé, and I forgot to urinate after that. I know the symptoms of a UTI, so I assumed the same. I was given a prescription for Bactrim without a urinalysis, and that did not make my symptoms any better. I visited an urgent care five days ago, and they did a urinalysis. They told me I still had a UTI and gave me Cephalexin. I just finished my Cephalexin today, and my symptoms are worse. I was in the ER yesterday, and then they said I did not have a UTI and that I needed to see a urologist because they did not know what it was. I have never had anything like this before. I take Linzess birth control pills and Macrobid for this same complaint. I have undergone CBC, CMP, CT scan, three urinalysis with cultures, yeast swabs, STI swabs, and BV swabs. Please help me.

I have some questions. Please answer them for a better understanding of your situation:

1. Do you still have symptoms like burning, frequency, and urgency feeling?

2. Did you have a fever in the past week?

3. For how long have you taken antibiotics, and which one?

4. Have you had similar symptoms in the past?

5. How long have you been sexually active?

6. Any other chronic illnesses like diabetes or hypertension or any medications?

7. Are any blood tests done, like CBC (complete blood count), RFT (renal function test), and urine test called urine culture?

8. How many days after antibiotics is the urine routine test that you have sent done?

Thanks for the reply.

I have burning and urgency sensation all the time. I have not had a fever. I took Bactrim for three days, then went to the urgent care, and got Cephalexin, and continued until today. I have never had symptoms like this before. It is affecting my ability to work and my mental health. I have had the same sexual partner for eight years and have not had sex since the infection. I do not have any other chronic illnesses. My CBC is done, and urinalysis with culture. I will attach those. The urinalysis I sent to you was a week after antibiotics.

I went through your reports (attachments are hidden to protect the patient's identity). I strongly feel it is a UTI (urinary tract infection) only. It is better to abstain from intercourse till this infection subsides fully. I will suggest you start with:

1. Tablet Levofloxacin 500 mg at bedtime for two weeks.

2. Tablet Lyser D (Diclofenac and serratiopeptidase) twice in the morning and at night for five days.

3. Syrup Citralka (Disodium hydrogen citrate) 10 mL thrice a day for two weeks.

3. Tablet Pyridium (Phenazopyridine) 1 tablet thrice a day for a week (orange color urine).

4. Drink plenty of water (so that 24-hour urine output is 0.6 to 0.79 liquid gallon).

5. Tablet Alfuzosin 10 mg at bedtime for two weeks.

Meanwhile, you can get a urine culture test and ultrasound abdomen with post-void residual urine as baseline tests.
