Hello doctor,

I had a bowel blockage with open surgery. I was discharged from the hospital with a possible diagnosis of diverticulitis. I can see blood and worms in my stool. What to do?

I went through your query.

I went through your reports (the attachments were removed to protect the patient's identity).

1) Bowel blockage is a emergency, and it must be corrected. Sometimes without surgery, but most of the time, it needs surgery. CT scan of the abdomen and pelvis with contrast must have been done considering your history to find out the cause.

2) Diverticulitis or diverticular disease can cause abdominal pain, diarrhea, bleeding, and sometimes bowel perforation (leak) but is less likely to cause mechanical bowel obstruction.

3) Colonoscopy would be considered, but if you have recent bowel surgery, immediate post-operative colonoscopy should be avoided for a couple of weeks, considering stitches inside.

4) I have seen pictures of your stools. This seems to be particularly digested food particles rather than worms.

5) Check your diet. If there is a high-fiber diet or raw vegetables, try to avoid that.

6) For confirmation, submit the stool sample for microscopic analysis (stool routine examination) next time. This would tell exactly whether these are worms or undigested food particles.

7) Stools with blood can be positive immediately after surgery or with medications you would be on, but yes, you need a colonoscopy.

Hi doctor,

Thank you for your answer.

I did a home fecal test last year in the previous month, which came back positive for blood. I am changing to a new PCP, which is effective. I hope for better communication and a PCP to get the necessary referrals for a proper diagnosis.

1) Your surgery was more than a year ago, and it is ok to perform a colonoscopy.

2) A fecal blood test is an excellent diagnostic test to check microscopic or occult blood in stools, and a positive test indicates colonoscopy.

3) As per recent USA guidelines, screening colonoscopy is recommended For any patient over 50 years of age, and recently, this age cut-off has been reduced to 45 years for some individuals.

4) I am hopeful that you should be referred for colonoscopy if not done yet up till now
