Hello doctor,

I take Vyvanse 60 mg once per day for adult ADHD inattentive type. I have to take it every day or I cannot function at work. Vyvanse has pretty much no effect on me though. I might lose my job. If I took a week off of Vyvanse, would it reset my tolerance? Because I might be able to request a week off of work and convince them that I will be back and ready after this adjustment. Would it work? (I have only been on Vyvanse for four weeks. Before that, I was prescribed Adderall 20 mg three times a day. I have been on ADHD medication since 2013.) I am asking this because my psychiatrist is apparently out of the office until the 20th. Please help.

I read your query and can understand your concern.

I would like to know if you are using any other substance of abuse like alcohol or tobacco. Also, I would like to know your symptomatology. Dexamphetamine has been an efficient medicine so far for ADHD (attention deficit hyperactivity disorder). But, if the response is not satisfactory, it can be stopped as you seem to have developed tolerance to the medications.

I would like to see your EEG (electroencephalogram) report.

I suggest the tablet Oxetol (Oxcarbazepine) 150 mg twice daily, along with the tablet Ativan (Lorazepam) 1 mg at night for a period of two weeks. However, please consult a physician, discuss with them, and start taking the medications with their consent.

You can review it with me along with your EEG report and detailed history for further follow-up.
