Patient's Query

Hello doctor,

I am a 19-year-old boy dealing with chronic pimples that have been affecting my confidence and causing rejection, especially in college drama circles. The consistent breakouts have taken a toll on my self-esteem and impacted my participation. I feel rejected due to my skin condition, and it is affecting my social interactions and pursuit of my passion for drama. I have tried various over-the-counter remedies with limited success. Can you provide guidance on more effective treatments or skincare routines specifically tailored for chronic pimples? I am eager for serious help that can address both the physical and emotional aspects of dealing with chronic pimples in a way that allows me to fully engage in my passions and social activities without feeling self-conscious.

Kindly provide your advice.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

The issue you are going through is acne vulgaris. They usually occur due to reasons including genetic susceptibility, where the condition runs within the family, due to hormonal fluctuations, which can happen during any age group and are most commonly seen among teenagers, due to changes in diet, due to sudden location changes, due to stress, due to sleep irregularities, due to any medical condition and any medications taken for any underlying medical conditions like antidepressants and anti-anxiety medications. Acne can also be seen in case of any sudden onset of using skin care products or creams, which triggers acne; consuming oily and fried food, sweets, red meat, full-fat milk, and dairy products can also increase the chances of acne. Treatment of acne is very important as long-standing, deep-rooted acne can result in the formation of acne scars and pigmentation. Squeezing, touching, and expressing acne may lead to acne scarring and pigmentation. However, timely intervention and procedures, along with medications, will show the best results.

I suggest you follow the below-mentioned instructions:

  1. It is important to use a face wash that consists of both glycolic acid as well as salicylic acid. It can reduce the oil secretion within the skin, which leads to acne. Make sure to use the face wash twice daily. Take two pumps, massage the entire face in gentle circular motions for one minute, and wash off.
  2. Clindamycin and Benzoyl Peroxide, along with Adapalene, can be used in gel or cream forms on the acne in order to reduce the oil secretion and reduce the bacterial contamination within the acne. Make sure to use it by dotting the acne and dabbing it into the affected area in the morning after using the face wash. The gel, Adapalene, is advised to be used at night on the acne after using the face wash.
  3. Doxycycline is an antibiotic that can be used for seven to 14 days to reduce acne. A tablet containing Doxycycline and Lactobacillus may be consumed twice daily after meals for seven days.
  4. It is always advised to take Pantoprazole 40 mg 30 minutes before meals as it will help produce chances of acidity from antibiotics.
  5. It is also advised to use multivitamins, which help in reducing acne. The best multivitamin that can be included should ideally consist of zinc and selenium.
  6. Avoiding oily and fried food, sweets, milk, and dairy products and consuming more fruits, vegetables, fiber, and at least three liters of water per day will help in reducing the chances of acne.
  7. You can also undergo below-mentioned medical procedures:

    1. I suggest you combine medical procedures with medications for the best result.

    2. Chemical peels: This is a very simple and superficial procedure in which The doctor applies a peeling agent, which is a solution on the entire face, leaves it for ten minutes, and cleans it up. This is usually done once every two weeks for six to nine sessions for best results. They have the capacity to reduce oil secretion as well as antibacterial properties to reduce the bacterial growth within acne.

    3. Laser treatment: carbon peel lasers are very helpful in reducing acne as well as acne marks. This is a procedure that is usually done once a month for four to five sessions and is of great significance in reducing acne and acne marks to a great extent.

    4. I would also like to advise you to avoid any skincare products like serums, creams, and sunscreens during the presence of active acne. Once acne has subsided, it is okay to start with a skincare routine and sunscreens. Ideally, for chronic acne, a basic skincare routine is sufficient.

A morning skincare routine can consist of the above-mentioned face wash, a vitamin C serum, and sunscreen. A silicone gel-based sunscreen can be used. A nighttime routine can consist of face wash, vitamin C serum, and a moisturizer. I would like to assure you that acne is very common among teenagers and may usually be present till the early thirties in case of genetic susceptibility to hormonal fluctuations and the other triggers and risk factors that I have mentioned above. However, acne can be managed and treated with medication and procedures along with dietary control. A holistic approach is very much essential for the treatment of acne and acne marks. Treating and managing them the right way, based on the details provided above, you may be able to regain your self-confidence and get along with your peers much better. Acne is not a serious condition or disorder but is absolutely common and a normal phenomenon during the teenage years, early and late 20s. It can be easily managed with the right medications and treatments. Apart from the same, acne will reduce as you reach your early 30s due to the reduction of oil secretion from underneath the skin, making the skin slightly more prone to dryness. Therefore, I would like to advise you and assure you that you should not worry and should be happy and positive while dealing with your peers.

I hope this has helped you.

Kindly follow up if you have more doubts.

    Thank you.
