Hi doctor,

I cannot maintain an erection while having penetrative sex. Although, I can stay hard as long as I want when I masturbate. I also go soft while trying to put a condom on. I used to watch porn daily but have not watched it in the past six months. However, I masturbate occasionally and have a very low sex drive.

There is nothing wrong with you in terms of physical disease. Erectile dysfunction and low libido are extremely rare at your age. Instead, I believe you are suffering from performance anxiety and negative sex conditioning. We must remember that erection occurs in two ways. One is psychological. When you think about sex or see nude images, you become aroused and have an erection. Physical stimulation is number two. When the soft, slippery surface of the vaginal walls stimulates your penis, it provides pleasant stimulation. When we develop a negative emotion or feeling toward sexual practices such as masturbation or porn, that negative emotion or feeling blocks our psychological pathway to arousal. The desire is diminished, anxiety takes over, and the erection is lost. Similarly, when you put a condom on, your entire focus is on the penis and its erection. Even a millisecond of thought about losing the erection disrupts the psychological arousal, and you lose it.

So you need to remove the pressure from the sexual act. There should be no feelings of guilt or anxiety about any sexual activity. Attempt to devote more time to foreplay rather than penetration. It will have several advantages:

  • It will divert your attention and anxiety away from the erection.
  • It will help you become more excited and stimulated, increasing your erection.
  • It will provide your partner with so much pleasure that she may be less concerned about having penetrative sex.

If you want to experiment, oral sex is a great option.
