Shortness of breath in a 22-year-old young lady (General Medicine)

Ramasamy Maruthiah Thevar., MBBS, MD GENERAL MEDICINE

Medical Case Details:

This patient till 2 days back was asymptomatic, suddenly developed shortness of breath, vague chest pain, palpitation, & numbness of both UL. She was brought to the ED the next day. In the present scenario of COVID-19, the possibility of COVID-19 was entertained. History, clinical exam, baseline lab investigations like CBC, LFT, ECG, RBS, CXR (done elsewhere) did not provide any clue to the shortness of breath. What could be the cause?

Pulm embolism can be a possibility.

Should get an Echo done to check for MVP- MR or pulmonary embolism (though less likely if saturations, BP, pulse rate are fine and normal.) If echo is normal then Anxiety disorder or other causes can be thought of.Thank you.

Sudden onset of breathing difficulty necessitates to rule out No.1 pulmonary embolism 2.acute heart failure 3.acute asthma/copd attack 4.acute pneumothorax. So I think do BNP levels if high perform echo ABG D Dimer Doppler for legs to be done. Then proceed according to findings
    Thanks
